New Jersey’s Community Care Program (CCP) replaced the Community Care Waiver (CCW) in 2017. While both initiatives were put in place by the Division of Developmental Disabilities (DDD) with similar objectives in mind, there are several key differences.

Key Differences Between the CCW and CCP

Program Structure

While the CCW was a standalone 1915(c) waiver for adults with disabilities, the CCP was integrated into New Jersey’s 1115 Comprehensive Medicaid Waiver. This change benefited New Jersey residents with disabilities, as it provided more flexibility in funding and the administration of services; however, many families were confused by the transition.

Whenever a significant change like this occurs, our local and state governments must do a better job of explaining how the transition will work, ensuring that all people who rely on CCW understand how to transition to CCP.

Eligibility

The Community Care Waiver’s eligibility standards included all adults with disabilities requiring institutional-level care. The Community Care Program integrated the same criteria; however, it became a dual system, differentiating between:

  • CCP – for those with the highest level of needs, including those requiring 24/7 support or group home residences
  • Support Programs (SPs) – For adults with disabilities who require some services but not full-time residential care

Unfortunately, the CCP waitlist is lengthy, sometimes requiring individuals to wait years before receiving the support they deserve. Support Programs allow these individuals to receive at least some assistance while waiting for their turn on the waitlist.

Case Management

DDD case managers oversaw the CCW, while the CCP is managed by independent support coordinators, helping individuals manage their services. This change was meant to provide more choices to individuals while mitigating biases in service recommendations. Unfortunately, this is not always the case; however, it is nice to see some improvements since 2017.

Residential Services

Prior to 2017, group homes and certain supervised apartments were covered. CCP provides similar coverage with added transparency. While this provides more sustainable and equitable funding for residential services, the limited availability of residential placements are a significant problem.
